There are many spicy levels at Nakamoto, but the most standard level and popular one is Mouko Tanmen (Miso Ramen with Spicy Mabo Tofu). The Miso soup itself isn't that spicy, but mabo tofu on top of it is. The noodles are thick and stir fried vegetables and pork are also added. If its your first time at Nakamoto, I would recommend you to order this, but that's if you do like spicy food.
